How can I use the Questionnaire activity?

The Questionnaire activity allows instructors to create and conduct surveys to gather information or feedback from students or other enrolled users. The Questionnaire does not include an option to grade submissions, it is strictly used for gathering information.
For example, you might use the Questionnaire activity to gather information from students to:

collect information to help you group your students for projects
- improve the course through mid-semester feedback
- learn about student preferences for course activities
Instructors can add the Questionnaire activity to a course through the "Add an activity or resource" link on the main page of their Moodle class.
Types of questions you may use include:
- Check Boxes
- Date Box
- Dropdown choices
- Essay box
- Numeric
- Radio buttons
- Scale
- Text box Yes/no
Responses can be exported in a variety a formats including .csv and others.
Using the Questionnaire in Moodle
